Update - Revised Forms - Required July 1st, 2010. Fannie Mae & Freddie Mac have revised the Uniform Residential Loan Application (Fannie Mae 1003/Freddie Mac Form 65). The revisions address the new Federal Housing Finance Agency’s (FHFA) loan-level origination data requirements, announced in FHFA press release on January 15, 2009. The revised form must be used with loan applications originated beginning July 1, 2010
